Real history
Oregon State Hospital opened in Salem as the Oregon State Insane Asylum on October 23, 1883. It was built on the Kirkbride Plan in the Italianate style, and it is the oldest operating psychiatric hospital in Oregon (Wikipedia). It was renamed Oregon State Hospital in 1913 (Wikipedia).
A narrow-gauge railroad and tunnels linked the buildings and were used to move food, laundry and sometimes patients out of public view (Wikipedia, Tunnels). The hospital took part in Oregon's eugenics program: an estimated 2,648 people were sterilized statewide between 1917 and 1983, the hospital's surgery center was the main facility for the operations, and 509 sterilizations were recorded at the hospital itself between 1918 and 1941 (University of Oregon, excerpting Diane Goeres-Gardner). On November 18, 1942, sodium fluoride roach poison was mistakenly used in place of powdered milk in scrambled eggs; 467 people fell ill and 47 died (Wikipedia).
The population peaked at nearly 3,600. The Oregonian's 2005 editorials on the hospital, including its thousands of canisters of unclaimed cremated remains, won the 2006 Pulitzer Prize for Editorial Writing (Pulitzer Prizes). The main hospital was rebuilt as a 620-bed facility; the first patients moved into the new Salem building in January 2011 (The Bulletin); the center of the Kirkbride building was kept and now houses a mental health museum. It was the main filming location for One Flew Over the Cuckoo's Nest (1975) (Wikipedia).
Ghost stories
These are reported legends, not verified events. The creepiness score (1–5) is Spook Atlas's own editorial rating, not a measurement.
Former employees and residents have reported doors closing on their own, wailing and footsteps, cold spots, and shadows in corners, especially around the 1883 "J" Building. Skeptics put this down to old buildings settling. Source: Willamette Heritage Center, Haunted Salem.
A story tells of a nurse who died in an elevator shaft and now walks the basement tunnels. The source itself calls it unproven and likely a myth. Source: HauntedUS.
